THE CONDITION · CERVICOGENIC
A headache generated by the neck, felt in the head. It stays on one side. Moving the neck brings it on. And it is one of the most often missed causes of a headache that has not responded to migraine treatment.

How it behaves
- Starts at the back or side of the neck. It spreads up and forward, often to the forehead or behind the eye.
- Always the same side. It does not swap.
- Provoked by neck position or movement — a long drive, a low monitor, sleeping awkwardly.
- The neck does not turn as far as it should. This is often old and goes unnoticed.
- Usually a steady, medium ache. It does not pound or throb.
- It can bring nausea and light sensitivity. That is why it gets called migraine.
The convergence that explains it
The top three cervical nerve roots (the nerves of the upper neck) share a relay station in the brainstem with the trigeminal system (the nerve that serves the face). So signals from the neck are felt in trigeminal territory — the forehead, the temple, behind the eye. This is not loose talk about pain spreading. It is a specific wiring overlap. That is why a neck problem is felt in the front of the head with full conviction.
Why migraine treatment disappoints here
Because the source is a physical part outside the brain, and the treatment is aimed at a brain disorder. People cycle through preventives. They get some benefit or none. Then they are labeled treatment-resistant. The label is wrong. The target was wrong.
The exam is what tells them apart, and it takes minutes. It checks how far the neck moves, soreness over the upper cervical joints, and whether holding the neck in one position brings on the headache. It is skipped all the time. That is the single most fixable gap in this whole area.
What drives it, beyond the joint
Steady load on the cervical extensors (the muscles that hold the head up) is the physical input. Two whole-body factors set how easily that turns into pain. The first is metabolic inflammation. It keeps the joint capsule (the sleeve around the joint) and nearby tissue breaking down instead of rebuilding. The second is sleep, which sets how sensitive you are the next day. The third is the work posture that makes the load in the first place. Unlike the other two, it is usually fixed by furniture and scheduling, not by medicine.
Frequently asked questions
How do I know if my headache is coming from my neck?
It stays on one side. Neck position brings it on. The neck moves less on that side. If all three are there, it is worth a proper exam, and a diagnostic block can confirm it.
How neck-driven pain is told apart from the other three sources at the back of the head is covered in pain at the back of the head.
Can a neck problem really cause pain behind my eye?
Yes, and it is common. The upper cervical and trigeminal systems meet in the brainstem. So signals from the neck are felt in the face and forehead.
Will physical therapy fix it?
It helps the physical part and how much load the neck can take. It is worth doing. When a set source of pain keeps going, a diagnostic block finds it and changes the plan.
The diagnostic block sets out how a block finds the source and who it suits.
Is this the same as tension headache?
No. Tension-type is on both sides and pressing. It does not stay on one side, and neck position does not bring it on.
Which neck pain and headache symptoms need the emergency room?
New neck pain with a headache that came on suddenly, especially after a fall, a car accident or a neck manipulation, goes to an emergency department the same day. Add a drooping eyelid or one smaller pupil, double vision, slurred speech, unsteadiness or new weakness, and the concern is a tear in an artery of the neck (cervical artery dissection), which can cause a stroke. Fever with a stiff neck cannot wait either. The slow, one-sided, posture-driven ache described above is an appointment, not an emergency. Learn more: the headaches that are not an appointment.

Neck-driven headache and unsteadiness travel together more often than either is asked about.
If migraine treatment has not worked, ask whether the neck was examined
It takes minutes and it is skipped constantly. That question is often the turning point.
