WHERE IT HURTS · JAW, EAR AND TEMPLE
A headache with ear or jaw pain most often comes from the temporomandibular joint (the jaw joint) and the chewing muscles around it. Night grinding is the usual driver. The pain spreads into the temple because the temporalis muscle spans the temple and attaches to the jaw.
Ear pain with a normal ear exam is one of the most reliable signposts in headache medicine. It points down, to the joint and the muscles in front of the ear.

Can TMJ cause ear pain when the ear is normal?
People show up again and again at primary care and at ENT (ear, nose and throat) with ear pain. The eardrum looks normal, and they are sent away. The temporomandibular joint sits right in front of the ear canal. It shares the same nerve territory. So joint and muscle pain feels exactly like ear pain. A normal ear plus a lasting earache is the finding, not a dead end.
WHAT POINTS TO THE JAW
- Clicking, popping, or the jaw catching when you open.
- Worse when chewing gum or steak, or after a long dental appointment.
- Morning soreness and flattened, sensitive teeth. That is the signature of night grinding.
- Tenderness when you press just in front of the ear canal while opening and closing.
- Pain that spreads into the temple, because the temporalis inserts there.
WHAT POINTS ELSEWHERE
- True hearing loss, discharge or vertigo. That belongs to ENT.
- Shooting, electric pain in the ear or throat set off by swallowing. That is a neuralgia, a nerve pain. It is uncommon but specific.
- A one-sided ache with a normal ear in someone over 50 who smokes or drinks heavily. Referred pain from somewhere else needs to be ruled out properly.
What causes teeth grinding at night, and why can’t you just stop?
Sleep bruxism, grinding in your sleep, is not a habit you can fix by trying, because it happens during brief arousals from sleep. That is the key fact, and it changes the treatment. The target is sleep quality and load, not willpower. Two biological inputs feed it. Broken-up sleep increases clenching tied to those arousals. And high sympathetic tone, the body’s fight-or-flight setting, raises resting muscle activity.
The third depends on your situation, and it is what people actually notice. It is a stretch of long, hard concentration or job pressure. That raises daytime clenching and worsens sleep at the same time. That is why this arrives in runs of weeks, not at random.
Frequently asked questions
Why does my ear hurt when my ear is fine?
Because the temporomandibular joint is right in front of it and sends pain there. A normal ear exam with a lasting earache is a positive finding. It points at the joint.
Can jaw problems cause headaches?
Yes, commonly in the temple, because the temporalis muscle spans the temple and attaches to the jaw. See the temple page.
Should I see a dentist or a headache clinic?
See a dentist for the bite, guard and tooth wear. See a headache clinic if head pain is the main complaint, or if there is a separate headache disorder underneath. It is not one or the other. Often both help.
Does a night guard fix it?
It reliably protects the teeth and helps the pain in many people. But it does not address the broken-up sleep driving the clenching. See sleep and headache.
How can you tell if a headache is coming from the jaw?
Look for the jaw signs. The joint clicks, pops or catches when you open. The pain is worse after chewing gum or steak, or after a long dental appointment. You wake with a sore jaw and flattened, sensitive teeth. Pressing just in front of the ear canal while you open and close is tender, and the ache spreads up into the temple.
Where do you feel a TMJ headache?
Mostly in the temple and in front of the ear. The temporalis muscle spans the temple and attaches to the jaw. So an overworked jaw is felt as temple pain. The jaw joint sits right in front of the ear canal and shares its nerve supply. That is why it is so often felt as an earache with a normal ear exam.
How do you get rid of a TMJ headache?
Treat what drives the clenching, not just the pain. Night grinding happens during brief arousals from sleep, so willpower cannot stop it. The targets are sleep quality and daily load. That includes stretches of intense focus or job pressure. A night guard protects the teeth and helps the pain in many people. A dentist handles the bite and guard. A headache clinic handles the head pain.
When is ear and jaw pain something other than TMJ?
When the ear itself is involved. True hearing loss, discharge or vertigo belongs to an ear, nose and throat doctor. Shooting, electric pain in the ear or throat set off by swallowing is a nerve pain. It is uncommon but specific. A one-sided ache with a normal ear in someone over 50 who smokes or drinks heavily needs other causes of referred pain ruled out properly.
