Headache in pregnancy · St. Louis

Most migraine improves. The threshold for checking still drops.

THE SITUATION · PREGNANCY

Most migraine improves in pregnancy, especially after the first trimester. This page exists for the headaches that do not improve, and the ones that start. That is because in pregnancy, the bar for getting checked is set lower on purpose.

What usually happens to migraine in pregnancy?

Roughly two-thirds of people with migraine improve during pregnancy. The change is usually clearest in the second and third trimesters. The mechanism is the same one behind menstrual migraine, in reverse. Estrogen stays high and steady instead of rising and falling. Attacks often return within weeks of delivery as levels fall again.

Migraine with aura improves less reliably. Aura sometimes shows up for the first time during pregnancy.

WHEN A HEADACHE IN PREGNANCY IS URGENT

  • A new or severe headache after 20 weeks, especially with high blood pressure, swelling of the face or hands, visual changes, or upper abdominal pain — this is assessed the same day.
  • The worst headache of your life, or a sudden thunderclap onset.
  • Headache with fever and a stiff neck, seizure, confusion or any new neurological sign.
  • A severe headache in the days and weeks after delivery, especially after an epidural or spinal. Both the low-pressure and the preeclampsia pictures occur after birth.
  • These go to obstetric assessment or an emergency department, not to a routine appointment.

The bar is low for a reason. Preeclampsia and cerebral venous thrombosis (a clot in a vein of the brain) both show up with headache. Both are time-critical. That is not alarmism. It is why the rule exists.

What helps with headaches during pregnancy?

Treatment options are truly narrower in pregnancy. Every decision belongs jointly to you, your obstetric team and whoever manages your headaches. Nothing on a website should settle a medication question here. This page will not try.

Here is what is worth knowing. The non-drug levers carry more weight than usual, because the drug options are limited. Sleep timing, regular meals and fluids do more work now than at any other time. And in some cases, interventional options such as a nerve block become relatively more appealing. That is because the exposure is local, not whole-body. That is a talk to have with the team, not a recommendation from here. Why local versus systemic matters.

Frequently asked questions

Will my migraines get better in pregnancy?

For most people, yes, especially after the first trimester. Migraine with aura improves less reliably.

Is a headache in pregnancy dangerous?

Usually not. But after 20 weeks the bar for getting checked is set low on purpose, because preeclampsia shows up this way. Do not wait it out.

What can I take?

That is a decision for your obstetric team and prescriber together. This page will not give a medication answer. Be wary of any site that does.

What about after delivery?

Attacks often return within weeks. A severe headache after birth, especially after an epidural, needs to be checked, not watched.

When should I be worried about a headache during pregnancy?

When it is new or severe after 20 weeks. That is most urgent with high blood pressure, swelling of the face or hands, visual changes or upper belly pain. The same goes for the worst headache of your life, a sudden thunderclap onset, fever with a stiff neck, a seizure, confusion or any new neurological sign. These go to obstetric assessment or an emergency department the same day, not to a routine appointment.

What does a preeclampsia headache feel like?

Do not try to judge it by how the pain feels. The rule is built on timing and the company the headache keeps. A new or severe headache after 20 weeks is checked the same day. That is most urgent with high blood pressure, swelling of the face or hands, visual changes or upper belly pain. Preeclampsia shows up with headache and is time-critical. So waiting to see whether it settles is the wrong test.
