WHAT COMES WITH IT · DIZZINESS
Headache with dizziness is most often vestibular migraine. It is one of the most frequent causes of repeat vertigo (spinning). Episodes last minutes to hours, often with little head pain. BPPV, low blood pressure on standing, the neck and medication are the other common causes. Sudden severe vertigo with neurological signs is an emergency.
Dizziness is four different complaints wearing one word. Which one you mean decides almost everything that follows.

What kind of dizziness do you have?
- Spinning — the room moves, or you do. That is vertigo.
- Light-headed — about to faint, drains away when you stand.
- Unsteady — balance is off when walking, no spinning.
- Foggy — spaced out, hard to focus, not really a balance symptom at all.
People use these words as if they mean the same thing. Doctors hear them that way too. That is how this symptom goes unsolved for years. Pick the closest one and lead with it.
Is it vestibular migraine?
Migraine is one of the most frequent causes of repeat vertigo. Episodes last minutes to hours, often with light and sound sensitivity. Here is the part that fools everyone: they often come without much headache at all. People get tested for inner ear disease for years while no one looks at the migraine history.
The clue is usually somewhere else in the history. Look for migraine in you or your family, motion sickness as a child, or the same triggers as your headache attacks.
OTHER COMMON EXPLANATIONS
- BPPV — brief spinning, lasting seconds, set off by rolling over or looking up. It is found and treated with head-position moves (positional maneuvers), often in one visit.
- Low blood pressure on standing — light-headedness on rising, not spinning.
- Cervical contribution — unsteadiness with a stiff neck that does not move fully.
- Medication — a long list, and worth reviewing before anything else.
URGENT INSTEAD
- Sudden severe vertigo with double vision, slurred speech, weakness, numbness, or being unable to walk.
- New unsteadiness with a severe or sudden headache.
- Vertigo with new hearing loss on one side.
- The red-flag list.
Why do headache and dizziness happen together?
The balance centers (vestibular nuclei) and the trigeminal pain pathways work together in the brainstem. So the same sensitized state that lowers the headache threshold (tipping point) also lowers your tolerance for motion. That is the biology. On top of it sit the same two drivers you can change as everywhere else on this site — sleep debt and unsteady blood sugar. There is a third that is specific here: the avoidance that dizziness causes. Moving less to avoid symptoms puts the balance system out of shape. That makes the next episode worse. That is why treatment usually includes step-by-step (graded) movement, not rest.
Frequently asked questions
Can migraine cause dizziness without a headache?
Yes, and it is common. Vestibular migraine often shows up with little or no head pain. That is why it is missed.
Is dizziness with a headache dangerous?
Usually not. But sudden severe vertigo with neurological signs is an emergency. The alert box lists what counts.
Should I see ENT or a headache clinic?
ENT if there is hearing loss, fluid from the ear or classic positional vertigo. A headache clinic if there is a migraine history and the episodes come with light or sound sensitivity.
What helps?
For vestibular migraine, migraine treatment. For BPPV, repositioning. For a balance system out of shape (deconditioning), step-by-step exposure. The label decides the answer. That is why naming the type of dizziness matters.
Should I go to the ER for headache and dizziness?
Go today if the vertigo is sudden and severe and comes with double vision, slurred speech, weakness, numbness or trouble walking. The same applies to new unsteadiness with a severe or sudden headache, and to vertigo with new hearing loss on one side. Without those signs, headache with dizziness is usually not an emergency and belongs in a clinic visit.
Why do I feel off balance but not dizzy?
Unsteady is its own complaint. Your balance is off when you walk, but nothing spins. When it comes with a stiff neck that does not move fully, the neck is often adding to it. Moving less to avoid the feeling makes it worse, because the balance system gets out of shape. So treatment usually includes step-by-step movement, not rest.
What else causes dizziness with a headache besides migraine?
Four causes are common. BPPV brings brief spinning, lasting seconds, when you roll over or look up. It is often treated with positional maneuvers in one visit. Low blood pressure on standing brings light-headedness when you rise. A stiff neck that does not move fully brings unsteadiness. Medication causes it too. That is why your list is reviewed before anything else.
