THE CONDITION · NEW DAILY PERSISTENT HEADACHE

New daily persistent headache is a headache that becomes nonstop within 24 hours of onset. It happens in someone with no major history of headaches, and it lasts more than three months. Secondary causes, those driven by another problem, are ruled out first. Imaging is usually appropriate.

A headache that started on a day you can name and never stopped. Being able to date the start is not a detail. It is the diagnostic rule itself.

What counts as new daily persistent headache?

A headache that becomes constant and never lets up within 24 hours of onset. It happens in someone with no major history of headaches. It lasts more than three months. What sets it apart is that people remember the day. Most headache disorders build. This one arrives.

It often follows an infection, a stretch of intense stress, or a surgical procedure. Often, though, no trigger is found at all.

WHAT MUST BE EXCLUDED FIRST

  • Raised or low intracranial pressure, the pressure inside the skull. Both can show up as a new nonstop headache, and both are treatable.
  • Cerebral venous thrombosis, a clot in the brain’s veins. Think of it mainly around pregnancy or with clotting risk factors.
  • A leak of spinal fluid, especially if it is clearly worse upright and better lying flat.
  • Infection or inflammatory causes, mainly if it followed an illness.
  • This is one case where imaging and more tests are usually appropriate, not optional. See when imaging is indicated.

Why is new daily persistent headache hard to treat?

It responds poorly to standard treatment. There is no established first-line therapy. That is the true picture, and there is no point dressing it up. What is worth knowing is that a real share of people improve over months to a couple of years. Some clear up entirely.

It also has two forms. One is self-limiting and fades over months. The other persists. No one can know at the start which one you have. That argues for treating it actively instead of waiting to find out.

How is new daily persistent headache treated?

Rule out the secondary causes properly, first and thoroughly. Then treat by the type it most looks like. Migraine features are treated as migraine. Tension-type features are treated that way. If the exam finds a neck or back-of-head (cervical or occipital) part, a block is worth it for what it tells us alone.

The same three drivers that keep headaches going matter more here than usual, because there is no drug to lean on. They are sleep, the metabolic terrain, and a life pattern with no recovery in it. When drugs underperform, those stop being add-ons.

Frequently asked questions

What is new daily persistent headache?

A headache that became nonstop within 24 hours of a start you remember. It happens in someone with no prior headache history. It lasts more than three months.

Will it go away?

For some people it fades over months to a couple of years. For others it persists. Both outcomes happen, and neither can be predicted at the start.

Do I need a scan?

Yes, in this case. It is one of the times when testing is truly called for. It is not just for peace of mind.

Is it in my head?

It is a recognized diagnosis in the international classification. It is not a leftover label for when nothing else fits. Being told it is stress without a proper workup is not an acceptable end point.

Why am I getting headaches every day all of a sudden?

Say a headache became constant within a day of starting, in someone who rarely had headaches before. Once it has lasted more than three months, that is the pattern of new daily persistent headache. It often follows an infection, a stretch of intense stress or a surgery. Often there is no trigger at all. Pressure problems, a clot, a spinal fluid leak and infection are ruled out first.

Is new daily persistent headache the same as migraine?

No. It is its own recognized diagnosis in the international classification. It is defined by a sudden start you can put a date on and a headache that never stops. What it feels like guides treatment. When it has migraine features, it is treated as migraine. When it has tension-type features, it is treated that way. A neck or occipital part on exam is worth a diagnostic nerve block.

What has to be ruled out when a new daily headache will not stop?

Four things come first. First, pressure inside the skull that is too high or too low. Second, a clot in the brain’s veins, especially around pregnancy or with clotting risk. Third, a spinal fluid leak that is worse upright and better lying flat. Fourth, infection or inflammation, especially after an illness. This is one case where imaging and more tests are part of the workup, not optional extras.
